Diabetes Revisited

A program that offers you a chance to get up to date on diabetes care

If you:

  • have Type II diabetes
  • were diagnosed with diabetes at least 1 year ago
  • have taken diabetes education workshops before, and
  • are a PCN patient

then you are eligible to join this free of charge, six-week class. The course will update you on healthy eating, dealing with depression, foot care, exercise, and medication use.

Program Eligibility: Spouses, partners and care-givers are welcome.
Class Duration: Five — 2 hour classes
