What is the St. Albert & Sturgeon Primary Care Network?

The St. Albert & Sturgeon Primary Care Network (PCN) is a group of family physicians and other health care professionals (e.g. dietitian, lactation consultant, mental health coordinators, nurses, pharmacist, etc.) working together to provide enhanced primary patient care.

The primary care focus of the PCN is on:

  • Access to Care
  • Chronic Disease Prevention and Management
  • Geriatric Evaluation and Management
  • Mental Health Support Services
  • Primary Care Maternity

Which clinics are part of the St. Albert & Sturgeon PCN?

Primary health care is the first point of contact with the health care system, where people receive care from family physicians, and from nurses, pharmacists, therapists and others for most of their everyday health care needs.
